As part of Cooper City Church of God Missions Appreciation Week, February 22-26, 2023, I will partner as a Service Provider for - MISSIONARY MEAL: missionaries are to be picked up at the church in Connection Central at 4:30PM and returned by 6:30PM for a meal at a restaurant or served at your home.

If you have been a meal provider in previous years, it is not assumed that you will do so again. Meal providers are assigned based on the timely return of this form; please do so as soon as possible.  Completion of this form in a timely manner does not guarantee to be a missionary meal host.
